First Time Visit

 

Thank you for letting us help take care of your Oral Health needs.

 

For your first visit, it is important that you provide us with a few details:

​

• Your complete Medical History. Make sure you can provide us with a list of all medications that you are on and any allergies that you might have.

• If you plan on using your insurance, please bring us your insurance card and a form of identification.

• If you do not have insurance, you are always encouraged to sign up for the Pearl Family Smiles discount program to be eligible for significant savings on your dental treatment.

• It is important for legal guardians to be present when the patient is a minor so that consent can be provided.

 

Please know that your scheduled appointment time has been reserved specifically for you. We request a 48 hour notice should you wish to cancel or reschedule your appointment. We know that sometimes emergencies happen, but please let us know ahead of time so we can help another patient take care of their needs.

A short note on Dental Insurance plans. Most Dental Insurance plans do not work like Medical Insurance plans. They do not provide benefits to restore your mouth should you need extensive work. Most of our patient's benefits are capped at $1,500 or less each year. We offer multiple different options to help you get the care you need without causing undue strains on your budget. Also, please realize that your insurance is a contract between your employer and your insurance company. While we can only help, you are the final guarantor for your Dental treatment costs.
